Free and Reduced Lanch Application forms are sent home with each student on the first day of school. If your child did not receive a packet, please downloan the forms below or contact the main office at his/her school.
If you would like to have your information shared with the specific other programs listed on Addendum A, please fill out that form and return it with the Family Application.
Husky Insurance Applications (Addendum B) should be mailed to the address at the bottom of the form.
