PLEASE BE AVDISED OF THE NEW POLICY ON PHYSICALS:
A student who wishes to participate in interscholastic sports must obtain a physician’s examination/physical performed within the past 12 months and documented by the physician on a form provided by Brookfield High School. This physical examination must be performed by the student’s own physician as BHS will no longer be offering physical examinations.
Any physical that is due to expire during the season must be renewed. There will be a 30 day grace periodfor renewal during which time the athlete will still be eligible to participate. If the athlete has not submitted a new physical to the school nurse before the expiration of the 30 day grace period, the athlete will become “medically ineligible” and will not be permitted to continue until a new physical has been performed, submitted and approved by the school nurse.
PLEASE NOTE: In order to be valid, all physicals must be signed by either a:
- Medical Doctor (MD)
- Doctor of Osteopathy (DO)
- Advanced Practice Registered Nurse (APRN)
- Physicians Assistant (PA)
Below you will find downloadable versions of the athletic forms to register for a sport. All sections of the forms must be completed and signed by the designated individual.
You will need to obtain an emergency card (this is not downloadable) from the Athletic Office which will need to be filled out and submitted as well.
Please be aware that athletes must maintain at least a 1.75 GPA in order to be eligible to participate, and all registration forms, medical clearances & pay to participate monies must be submitted prior to tryouts.
